Age-appropriate transition assessments should be the driving force of transition planning. These assessments should begin early in the student’s educational career and should be an ongoing process, as individual needs, interests and preferences may change often.
It truly takes a village! Information provided by ALL members of a student’s IEP team is crucial for developing appropriate transition plans that take into account the individual as a whole.
